I hereby authorize Christian Homes, Inc. to thoroughly investigate my references, criminal record, work record, education and other matters related to my suitability for employment and, further, authorize my former employers and any other third party to disclose to Christian Homes, Inc. any and all letters, reports, and other information related to my work record, without anyone giving me prior notice of such disclosure. In addition, I hereby release Christian Homes, Inc., my former employers, and all others from any and all claims, demands, cause of action or liabilities arising out of or in any way related to such investigation or disclosure, including but not limited to liability for slander, libel and interference with employment relationships, contracts, or business advantages. I understand that I will be required to undergo a pre-employment drug test and pre-employment physical examination in accordance with this employer's policies. I understand that this employer will conduct a criminal background check on me in connection with this application. I waive any legal requirement that the employer provide any notice to me regarding investigative reports or records given or received in connection with this application.
